We would appreciate it if requests for home visits be made as early as possible after 8.30am.

Patient Registration


Our Practice area covers the triangle between Redruth, Truro and Penryn. We care for Patients from all the villages within this area. We can register new Patients outside of our practice boundary, however, we are unable to provide home visits should one be required (you will be informed of this at the time of registration). We aim to provide a full range of family care in a  compassionate and professional way.

All Patients who register with the Practice are registered with a named GP who will be responsible for their overall care at the Practice. However, you do have the right to express a preference to receive services from a particular Practitioner and this will be recorded in your medical notes.

Whenever possible, a continuing problem is best dealt with by the same Doctor.

All new Patients are required to complete a GMS1 form (available from Reception or by clicking on the link below). If you would like to have a    ‘ get to know you’ appointment, please ask at Reception. This is recommended if you have any health concerns or are on regular medication.

Registration Form


New arrangements have been introduced to give patients greater choice when choosing a GP practice. Patients may approach any GP practice, even if they live outside the practice area, to see if they will be accepted on to the patient list.

The new arrangements mean that practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits. This is voluntary for GP practices.

The practice will only register patients without home visits if it is clinically appropriate as it may not be in your best interests or practical for you to be registered in this way. This also applies to patients who move address and wish to remain registered with the practice. You will be registered as an "Out of Area" patient.

If accepted, you will receive full medical services at the practice, but if you should need a visit you will need to contact us as normal. We will establish whether a visit is necessary, and if a visit is required we will inform you to contact NHS 111.

NHS 111 hold a directory of practices who provide visits to patients who live outside their practice boundary. NHS 111 will let you know which practice provides this service in your area and give you their contact details to request a visit. You will need to explain to the practice that you are registered at Chacewater and Devoran Surgeries, live outside the practice area, and that you require a home visit.
